Instructions:

- Click “Start your claim” to submit a new claim
- Upload your supporting documentation (purchase receipt) and click “Confirm my purchase”
- Enter all required information
- Enter your contact information
- You will go through an email verification process
- Agree to the program Terms & Conditions and Privacy Policy
- Click “Submit your claim” to receive your claim reference number
